Gall Bladder is an organ in our alimentary canal and acts as a reservoir for the bile. Bile is a yellowish, brown containing water, bile acids, pigments mainly bilirubin which is made from substances present in haemoglobin and blood pigments. Bile is secreted by the liver and is stored in the gall bladder. Bile helps in digestion. When the food is chewed, salivary glands step up secretion of the enzyme ptyalin breaking up carbohydrates into smaller molecules maltose and glucose. Food passing through the oesophagus (food pipe) reaches into stomach. Hydrochloric acid and enzyme pepsin is poured. It enters, into duodenum where digestive juices from pancreas are received. Bile breaks up fat globules.
Gall bladder concentrates the bile stored in it and is liable to infection either from the intstine or liver or the blood stream. Gall stones are formed in the gall bladder when these obstruct the hepatic or common bile duct, bile cannot escape from the liver and obstructive jaundice occurs. Liver produces about 700 to 1200 ml. bile per day. Urine is of yellow colour and stools are clay coloured with undigesicd fat.
In cholecystitis, due to inflammation of the gall bladder there may be epigastric pain, tenderness, nausea, vertigo, flatulence. Gall bladder infection takes place due to the infection in the teeth, tonsil or appendices passing into the blood.
These may be pure cholesterol stones, or pure pigment stones or of mixed type. Gall stones are hard and are generally of small sizes like gram and one or two may be of larger sizes. The way of expulsion of stones is through intestines and rectum. Patient generally suffers from dyspepsia. Eggs, cream and animal fats are avoided which may aggravate colic especially at night. Sudden acute pain erupts in the epigastrium or gall bladder region. The painrediates to right scapula or shoulder. During voilent pain; restlessness, sweating, nausea and vomiting may occur.
