9.   When your hand refuses to write

Writers cramp as the name suggest is something to do with writing. In this disease when a person attempts to instrument, the hand and finger muscles contract orextend halting the action or causing the abnormal posture of hand or wrist. Patient adopt a particular posture to overcome the defect. Some patient notice that the hand freezes during an attempt to move across the page and describe the hand as having “a mind of its own”. As soon as the writing instrument is removed from the hand the muscle relax.
This disease may start at an age of 15 years. At this time symptom may be of discomfort in writing or the patient starts taking much time for a given paragraph. As the disease advances the condition of patient gets worse.
In writers cramp abnormal posturing of the hand and forearm occurs. After some time, abnormal posturing occurs during other task such as applying cosmetics, shaving or using table cuttery. In this disease patient learns to use the other hand for these tasks.
But after few months he feels the same problem in that hand also. These task specific muscular contractions. Which produce abnormal posture can be found in barbers, tailors, violinist and person doing work with telegraph or computer key boards. In this disease when the hand is used for a skilled, occupationally related functions these muscular contractions occure. The strength of hand and fingers remain same and patient can do all other work as before. According to recent work this disease is due to abnormal processing of sensory input from the affected extremity during the work in basal gangra, a part of brain responsible for coordinated and harmonious working of all organs of body during fine work such as writing etc. In writer’s cramp basal ganglion gets affected and muscles of hand which is responsible for writing get contracted. It can be known through electro-myography.


SYMPTOMS OF WRITER’S CRAMP :


1. Difficulty in writing due to discomfort/pain in hand and fore-arm.
2. Tremor/shaking of hand during writing/holding pen in using instruments in case of workers.
3. Writing becomes disterted and untidy after a few sentences.
4. Abnormal posture of finger/thumb/wrist/hand during writing.
5. Difficulty in writing fast/hand freezes while writing
6. During writing holding pen/pencil tightly and it may fall during writing.
7. These above symptoms get aggravated by anxiety excitement and before strangers.
If anyone is suffering from any of the above symptoms he should consult neurophysician for proper diagnosis. People take it as psychiatric disorder or weakness of hand or care lessness of patient which is not correct.

Similar contractions can occur at other sites other than hand e.g. a part of face, tongue, neck etc. This is called focal dystonia. Apart from Basal ganglion dysfunction other causes may be environmental toxins or neuro toxic drugs.
Treatment for this disease is available in allopathy and homoeopathy. IN allopathy injections of botulinum toxin ( a neuro muscular paralyzer) into the involved muscles are sometimes helpful but functions ususally remain inpaired. This injection works for few months and is very costly.
In homoeopathy medicines given according to patient’s symptom and improvement begins within 02 weeks but patient should take the full course for complete recovery. Most of the patients get cured in six months but if the Disease is from many years and patient has taken much medication treatment for more than one year needed. Added benefit is very low cost. Few physicians advice for complete rest of hand for few months
